Then, simply complete the Questionnaire and bring it with you when you come in for your first appointment. If you prefer to fill it out when you get here, that's OK, but please arrive 20-30 minutes before your scheduled appointment.
Click here to download the Diet Diary (two 8.5 x 11 pages). This is the best method of tracking food intolerance or sensitivity. The patient writes down everything (including toothpaste, candy, gum, medications, etc.) that they put in their mouth, and records symptoms and any medications that are taken for the symptoms.
